Summary: You agree to share your information with us in order to participate in our program. At any point in time, you may request that any and all of your data be deleted from our systems.

PLEASE read this ‘Data Agreement’ before receiving services from the Dignified Work Program (DW). This agreement governs what information DW collects from you, stores, and shares. You can choose to The DW is available only if you (the “party”) agree to the Terms of Use set forth below. By marking ‘I agree’ on the Eligibility Assessment, the party signifies their agreement to these terms and are bound to them.

Data Agreement

This agreement establishes the terms and conditions under which the DW can acquire and use data from the party.

  1. DW requires the party to provide the following information:

    1. the party’s full name, 

    2. email address,  

    3. phone number,  

    4. residential zip code,  

    5. current employment status,  

    6. current or most recent annual income,  

    7. educational attainment,  

    8. number of dependent children 17 years or younger,

    9. age

      In addition, the party may be subject to background check at the time of hire. This information will be stored in DW data systems (defined below) and used by DW coaches, representatives, and/or staff to contact the party about matters related to their participation in DW services, content, or platforms and to evaluate the participant’s eligibility for the program.

  2. The party will have the right to request their data relating to their enrollment in DW. The party must submit a data request by email to support@dignifiedwork.org using the subject line: Data Request, and include in the body of the email:

      DW will have thirty (30) days to reply to the data request. 

  4. The party reserves the right to have their data deleted from the DW data system. The party must submit a data deletion request to support@dignifiedwork.org using the subject line: Data Delete, and include in the body of the email: 

    1. Date of request 

    2. The party’s full name 

    3. The party’s enrollment date 

    4. The data the party wishes to delete
      DW will have fifteen (15) days to reply with a break out of the data to be deleted. The party will then need to confirm the deletion of their data, whereby DW will have fifteen (15) days to delete the data from the data systems.  

  5. The party will have the option to provide standard demographic information, defined as the party’s gender identity, race, ethnicity, and military service. The party is not required to provide this information, and may request any information they do provide to be deleted following the Data Delete terms above. 

  6. The party will have the option to answer questions in the “Bridge to Strength Assessment”. The party may choose to opt out of answering these questions. In opting out, the party understands they cannot receive Mobility Mentoring Coaching. 

  7. The party is required to notify DW of any changes to their name, phone number, or email while participating in DW services, content, or platforms. This is to ensure DW can contact the party for matters related to their participation in DW services, content, and platforms.  

  8. DW will not release your name, or information that could identify you, nor will Dignified Work present the results of data analysis (including maps) in any manner that would reveal your identity. 

  9. DW will not release individual addresses, nor will present the results of data analysis (including maps) in any manner that would reveal individual addresses. 

  10. DW shall comply with all Federal and State laws and regulations governing the confidentiality of the information that is the subject of this Agreement. 

  11. DW will not share any contact information with a third party without explicit permission from the party.
